We will develop a treatment plan just for you; and carry you through the process that brings your skin to the level of health you have always desired. In order to accommodate our patient’s needs we ask half of the cost of the procedure to be paid at the time of scheduling and the remaining balance to be paid by the day of the service and prior to the date of the treatment. Missed appointments or cancellations of the procedure less than two business days prior to the schedule time will result in forfeiture of half of the procedure cost. When a patient fails to call within the 48 hours, he or she will be charged $25 cancellation fee. If the patient calls within 24 hours prior to the appointment date and reschedules, the charge will be waived.
As a courtesy, our practice will file insurance claim. However, it’s our patient’s responsibility to supply our office with the current and correct insurance cards at the time of service for every visit. If the patient cannot present a correct insurance card, payment is expected at the time of service.
